HOME HEALTH SERVICES UNIT
Within the scope of the Regulation on the Provision of Home Health Services by the Ministry of Health and its Subsidiaries, the examination, examination, analysis, treatment, medical care and rehabilitation of patients who need home health services are provided to our patients by our expert team in their own homes, under the coordination of Istanbul Provincial Health Directorate Public Hospital Services Presidency-5; In addition, patient relatives are given training on the issues to be considered in patient care and treatment.
Our team, which has served 26,605 patients since the day it was founded, continues to provide home health services to an average of 1,400 patients per month with 6 physicians, 13 midwives/nurses/health officers, 3 secretaries, 7 mobile teams and call reception-coordination teams. Our team aims to reach more patients by growing and renewing itself day by day.
What is Home Health Services?
Home Health Teams affiliated with our hospital; It aims to provide health services at home to socially disadvantaged, sick, in need of care, disabled and elderly individuals residing in Bayrampaşa, Fatih, Zeytinburnu and Sultangazi districts.
With Home Health Services; In addition to providing medical treatment and medical care, the aim is to raise awareness about the disease, to provide health and social support in the home environment, and to educate patients and their relatives about their diseases. With health services taking patients to their homes; It is aimed to prevent congestion that may occur in hospitals, reduce the length of hospital stay, and thus reduce the difficulties experienced by patients receiving home health care in accessing health care.
What are the Health Services Provided within the Scope of Home Health Services?
-Examination, examination, analysis, treatment, medical care and wound care services in the patient's home environment, within the framework of the diagnosis and planned treatment.
-Prescribing drugs whose long-term use is documented by a health report, provided that the special regulations in prescribing the drug are reserved.
-Assisting in preparing reports regarding the use of medical devices and materials.
-Informing the patient and his family about the duties they can undertake during home care and about the disease and care processes.
-Providing services such as training and consultancy to the patient regarding medical devices and equipment that he/she should use at home.
- Directing the transfer of the patient to the relevant health institution and/or from the health institution to his home, when deemed necessary.
-Providing training to patients and their relatives about the care of the inpatient.
-Renewal of patient reports.

How Can You Apply for Home Health Services?
Patients or family members who want to receive home health care can apply by calling the National Call Center within the Ministry of Health at 444 38 33 (444 EVDE) or they can reach our unit by calling 0212 567 67 05 [67 04].
The personnel on duty at the call center is the person requesting the service. Initiates the necessary procedures by having a conversation with the Following the first meeting, the person requesting service is interviewed by the health personnel at the Istanbul Home Health Coordination Center and the patient is directed to the relevant hospital's teams.

The home health care treatment plan is created after consultation by the physician with other physicians who plan the treatment of the people who will receive home health care, if any.
EMERGENCIES that occur except for the current conditions of the people who will receive home health care are not within the scope of duty of the unit.In case of emergency, 112 Provincial Ambulance Service should be called or directly applied to the Health Institutions Emergency Service.
In what cases is the service terminated?
If the patient no longer needs home health care, the patient is cured, the patient dies, the individual receiving patient care at home or his/her relative requests the termination of the service, the patient receiving home health care moves from the current location for any reason (The service can continue after the transfer if the patient who wants to continue receiving service notifies the Health Services Commission of this situation). provided), the service is terminated.
